GYNECOMASTIA IN MEN
Gynecomastia is the pathological increase in the volume of one or both breasts in men. The causes may be: hyperdevelopment of the mammary gland (due to hormonal, iatrogenic or tumor causes) and/or an increase in fatty tissue in the area (due to obesity).
To reduce gynecomastia in men, a surgical intervention is performed that consists of eliminating the glandular excess. Through a small incision around the areola, excess glandular breast tissue is removed, as well as excess fat and skin. As a result, a firm chest with a flat appearance is achieved.
However, in some cases this incision can be avoided and the procedure performed alone or with liposuction.
The intervention to eliminate gynecomastia does not require hospitalization, since it is usually performed under local anesthesia or under sedation. After surgery, the chest is bandaged to promote recovery and help tissue retraction.

A breast surgery usually lasts between 1-3 hours, depending on the case and the type of intervention. Normally a breast augmentation usually lasts 1 hour and a half, unlike a mastopexy that usually lasts longer.
The bulging chest in men (Gynecomastia) can be treated in two ways or with a combination of the two surgical techniques: if the problem is excess fat or pseudogynecomastia, a small liposuction of the pectoral area is carried out through a very fine cannula. The scars are only a few millimeters long and over time they will be imperceptible to the eye. On the contrary, if there is an excess of glandular tissue, which is known as true gynecomastia, the excess mammary gland is removed through a small peri-areolar incision that is concealed on the edge of the areola itself, also allowing the surgeon to reduce excess skin if present.
It is generally performed using local anesthesia and sedation. However, in some cases it is preferable to use general anesthesia. The surgeon will make an assessment, together with the anesthetist, about the best type of anesthesia for the patient.
The mammary gland or the fat do not usually regenerate if they are extracted correctly and without leaving residues.
7-14 days after the intervention, the patient will be able to return to her daily routine. However, it will depend on the type of intervention carried out and the work to be done. The guidelines for returning to physical exercise will be established with the surgeon according to the type of activity.
It is necessary to fast for at least 6 to 7 hours (no food or drink) before the time of surgery. You should also avoid wearing metal in direct contact with the body (such as earrings, chains, watches, rings, etc.) and go with your chest and armpits shaved 48 hours before surgery. Finally, NSAIDs or other medications that influence coagulation should be avoided.
A blood test, an electrocardiogram and in some cases a chest X-ray will be needed. With all this and depending on the age of the patient or the familiarity of some pathologies, a breast ultrasound and hormonal tests will be requested.
Before surgery, it is recommended not to smoke or take medications that can alter blood coagulation (such as some pain relievers or some natural products). Also, in patients who usually take anticoagulants, it will be the doctor who will indicate the suspension.
Depending on the type of technique or surgery, the postoperative period may vary, but in general the discomfort after gynecomastia correction is similar to stiffness or itching and is easily controlled with analgesics.
For a month you must wear a specific vest. In some cases, the application of cold through Hilotherapy will be recommended.
A good postoperative period, respecting the guidelines, helps to avoid future complications and will guarantee a faster recovery and better results. Tobacco consumption will influence the scars, also increasing the risk of hematoma.
The treatment of gynecomastia is ambulatory or in a day hospital regime and does not require hospital admission.
During the first week after chest surgery, the best position to sleep is face up, keeping the head higher than the thorax. In addition, for greater comfort and to facilitate this posture, pillows can be used. With all this, the surgeon will indicate when you can sleep on your side again.
The result begins to be appreciated from the second week after the intervention, once the inflammation begins to go down. Although it will improve over the next three months, you must wait for the maturation period of the scars to elapse, which is between 6 and 12 months.
Nipple sensitivity is not usually altered after an intervention for gynecomastia. Even so, in some cases the sensitivity increases or decreases temporarily, gradually recovering its normal state.
Normally it is recommended to wait between 2 and 3 weeks to resume physical exercise, which should be smooth and progressive. However, the doctor will set the tone for returning to daily routines after surgery.
